Base of thumb osteoarthritis
One of the most common referrals that we see for the hand, is for pain at the base of the thumb. This pain is often caused by osteoarthritis of the 1st carpometacarpal joint (1st CMC joint for short) which is the joint at the base of the thumb. It is so common, that it is estimated that 1/10 men and 3/10 women, in their 50s and 60s, may have osteoarthritis in their basal thumb joint and unfortunately this number continues to increase with age.
For many this pain can be a mild ache that comes and goes, but for some the pain is severe or persistent, impacting day-to-day function, comfort and even the ability to work. Things like opening jars or hanging the washing on the line, become tests of endurance, as pain and weakness make what was once a simple task, an unbearable one.
Fortunately, there is help and this is where your hand therapist steps in. Current research shows that suitable and timely treatment for thumb osteoarthritis is effective in reducing pain, increasing strength and also helps improve function of the hand.
For many, this may mean getting back to the things you enjoy doing or being able to continue working for longer. For some it may also mean avoiding painful and expensive surgery.
Your hand therapist will work with you to provide an accurate diagnosis and then to determine how the arthritis is impacting your life. This will allow the therapist to tailor an approach to therapy, that will best address your individual needs. Treatment will often include a combination of the following:
· Splinting – Custom made or specifically fitted to provide stability and support to the thumb.
· Assistive equipment – Up to date advice on available technology and tools to make tasks easier and protect our joints for longer.
· Joint protection – Strategies and modifications which address how you use your hands, to better take care of them.
· Exercise program – To improve and maintain thumb mobility, strength and stability.
· Massage and mobilisation – To help relieve pain and increase ability to use the hand.
